LAYAWAY CONTRACT

Please read the terms below carefully and make sure you understand them before entering into this
binding contract. Your first payment will serve as verification of your understanding and acceptance of
this contract.

Layaway deposits are preferred via PayPal. Please send all payments to me, Shelli Heinemann at PayPal
seller ID info@potbellybears.com. If paying by personal check, money order or cashier's check, please
make payable to Shelli Heinemann and mail to me at my studio address: 568 Manzanita #6, Chico, CA,
95926. NOTE: International customers must use PayPal for all layaway payments.

IMPORTANT: Layaway deposits are non-refundable, and items purchased using layaway are
not returnable.

-- You have agreed to purchase from me, Shelli Heinemann, an original POTBELLY work called:
__________________________. I have sent you an eMail invoice which includes the full retail price of your
bear ($__________;) any applicable sales tax ($_____;) and shipping and insurance charges to your
specified shipping address ($_____.) Your total is: $__________. There are no other fees.

-- You agree to make a total of two (2) payments for your bear on or before the dates specified below.

-- Deposit #1 equaling one half (50%) of the total due, or ($__________), is payable no later than
midnight PST five (5) days after invoicing, on _______________. You may pay with PayPal (preferred;)
personal check; money order; or cashier's check. International buyers must use PayPal.

-- Deposit #2 equaling the remaining one half (50%) due, or ($__________), is payable no later than
midnight PST thirty (30) days after invoicing, on _______________. You may pay with PayPal (preferred;)
personal check; money order; or cashier's check. International buyers must use PayPal.

-- Both payments must be received and your item paid in full before it will be shipped. If for any reason
you have difficulty making a payment, please contact me before your payment becomes past due to
discuss a mutually agreeable solution.

-- Late or non-payment will result in the cancellation of this contract and the forfeiture of all deposits
already paid.
